How to make a claim with Allianz Worldwide:
- To make a claim with Allianz, firstly we must look at the
type of treatment you are claiming for. Certain medical
treatments will require 'pre-authorization' with Allianz eg.
Hospital treatment.
- Pre-authorisation is getting approval from your insurance
company, prior to receiving your treatment. The policy holder
will need to complete a form and a 'Treatment Guarantee' will
be issued.
- When claiming for treatments that don’t require
pr-authorization, the policy holder will pay for treatment and
complete a claim form for reimbursement.
- In the case of a medical emergency, you do not need
pre-authorisation.

Which treatments need pre-authorisation with Allianz Worldwide?
Pre-authorisation is getting approval from your insurance company,
prior to receiving your treatment. The policy holder will need
to complete a claim form and a 'guarantee of treatment' will be
issued.
Allianz members can go online to view their policy benefits, or speak
with an Allianz consultant via the 24 hour Allianz Helpline to
confirm which treatment they are entitled to and which
treatments will require pre-authorisation.
The following treatments require pre-authorisation with Allianz.
- All in patient treatment
- Diagnostic scans: MRI (Magnetic Resonance Imaging), PET
(Positron Emission Tomography) and CT-PET scans
- Maternity treatment: routine maternity care and treatment
for pregnancy complications
- Home nursing
- Palliative care and long term care
- Cancer treatment
- Outpatient occupational therapy
- Outpatient rehabilitation treatment
- Medical Evacuation and Repatriation: emergency air
transportation, repatriation to home country, expenses for
accompaniment of one person, repatriation of mortal remains.
Please note: Allianz classes treatment as '1' or '2':
- If treatment guarantee is not obtained, Allianz has the
right to decline the claim.
If Allianz deems the
treatment medical necessary, only 80 percent of the entitled
treatment will be reimbursed.
- If treatment guarantee is not obtained, Allianz has the
right to decline the claim.
If Allianz deems the
treatment medical necessary, only 50 percent of the entitled
treatment will be reimbursed.

How to obtain a Treatment Guarantee from Allianz Worldwide
- Download a Treatment Guarantee form from the Allianz
website.
- Have your doctor or physician complete the relevant
sections of the Treatment Guarantee form.
- Complete the form and scan and email to:
medical.services@allianzworldwidecare.com Or you can fax to:
353 1 653 1780 or post to the address shown on the form.
- It is important that you send your completed form 5 days
prior to treatment, to avoid any delay in receiving your
treatment.
- If your treatment is scheduled within 72 hours, customers
can call the Allianz Helpline to submit your Treatment
Guarantee over the phone.
Please note: You must submit a treatment guarantee prior
to receiving treatment. If a treatment guarantee is not
obtained, Allianz may decline the claim all together or
reimburse only 50 to 80 percent of your treatment.

How to claim for treatments that do not require pre-authorisation
To claim for treatment that does not require pre-authorisation (such
as outpatient or dental treatment):
- Download a Claim Form from the Allianz website and bring
this with you to your medical appointment.
- At your medical appointment you will need to pay for your
treatment.
- Ask your doctor or health care provider to submit an
invoice which states your diagnosis, medical treatment
received, and the medical fees charged.
- Complete the Claims Form (you will complete section 1-5 and
the medical provider will complete 6-7).
- Attach original receipts or any documentation requested.
- Send the completed claim form to Allianz (address is shown
on form).
- An email will be sent to notify the customer of receipt of
claim.
- Fully completed claim forms are processed and paid to your
bank within 48 hours.
- Allianz will notify you via email within 48 hours if
further information is required (this will incur additional
time to process your claim).
Please note: You can track your claim using the Allianz Online Services.

How to preauthorize treatment in the event of an emergency
- In the event of an emergency, a Treatment Guarantee is not
required.
- If you require emergency treatment, you must contact the
Allianz Helpline within 48 hours of the accident or health event.

treatment details, treating doctor and hospital.
- Allianz will arrange a direct settlement if possible with
the treating hospital or medical facility.
